Building use form
If you would like to use the building and/or grounds, please complete the appropriate request form.
There are two different forms available:
Praise Ministries Building Use Form – for official PCF ministries, meetings, or activities.
Personal Building Use Request Form – for all other personal or non-ministry requests.
All requests must be submitted no later than one week prior to the requested date. You will receive an email from the PCF Office if and when your request has been approved. This process helps ensure there are no scheduling conflicts with other ministries or groups already approved to use the building.

Sunday school registration form
If your children are planning to attend Sunday School in the fall, the registration form is due each year in August. Fill out the form below and place into the white drop box next to the church office.

Need prayer or want to join our Praise Prayer Chain? Fill out this form and we’ll add you to our prayer chain which shares prayer requests of our PCF community . At the bottom, you can add a prayer request that we’ll share (or can be kept private) with our prayer warriors throughout the church.
